Cancellation Policy:
We realize that scheduled appointments may need to be changed; however, we require 24-hour notification of canceled or rescheduled appointments. This allows us to offer the time to another patient. If you fail to cancel without 24-hour notice resulting in a same day no show, we reserve the right to charge a $50.00 cancellation fee. This fee is not submitted to insurance, and it will be your responsibility to pay.
Late Policy:
If a patient arrives more than 15 minutes late for an appointment; the appointment may need to be rescheduled. This is to ensure that the patients who arrive on time do not wait longer than necessary to see the provider. You may be given the option of another appointment on the same day, if one is available. We will try to accommodate late-comers in the best manner possible but cannot compromise on the quality and timely care provided to our other patients.
Forms:
When requesting a form to be filled out, we ask for 2-4 business days for the form to be completed.
Medication Refills:
Please allow 48 business hours for a refill request to be filled. We ask that all refills be requested through the patient portal. Depending on the reason for the medication, follow-up appointments are required for the medication to be refilled.
Well child visits:
We follow the American Academy of Pediatrics guidelines for well child visits. For patients 3 years and older, our policy is that all patients have a well child visit annually.
For children younger than 3 years old, visits are at Newborn, 2-week,
1-month, 2-month, 4-month, 6-month, 9-month, 12-month,
15-month, 18-month, 2 year and 21/2 years.
Our well child visits are the same as a “sports physical”.
